R&L Heating & Air will examine and report the condition of all heating and air conditioning systems at the time of service by providing a copy of the service work order and invoice for work to be completed
No repairs will be started without the request and approval of supervisor or owner.
Our service personnel will extend every effort to advise and direct you to the best possible source for other needed parts or repairs.

(Please fill in the "Location of Equipment" lines ONLY if the equipment is located somewhere except your current address.)
- For the lines labeled – System#1, System#2, System#3 and System#4:
Please fill in your Make, Model No, Ton and Serial Number of your furnace or air conditioning unit.
(Note: We can fill in your System information for you on our first on-site visit if you are unable to locate this information.)
- Special Instructions – fill out this area for any special instructions that you may need to alert us about.
- Condensate Disclaimer Authorized Signature line. (Required)
Please type your first and last name to acknowledge that you agree with our Condensate Disclaimer. This line must have your name typed into it.
- Terms and Conditions of Agreement Initials (Required)
Please type in your two letter initials to acknowledge that you agree with the Terms and Conditions of this agreement. (The box to enter your initials is located just under Method of Payment.)
- Method of Payment (Required)
Choose one of two options "Bill my Credit Card" or "Check Enclosed".
